鼻咽癌

鼻咽癌是頭頸癌的一種，
是發生於鼻咽（頭部中央，
鼻孔與吊鐘上方之間，
前為鼻腔鼻竇，下通口腔部位）
的惡性腫瘤¹。

另外，男性的發病率比女性高約3倍¹，在2021年，男性的鼻咽癌新症有558宗，而女性的新症數目則為214宗^2,3。

鼻咽癌的發病高峰年齡為50-60歲¹，在2021年，共有211名年齡為50-59歲的鼻咽癌新症患者⁴。鼻咽癌主要與EBV有關，但亦有一些研究表明，少數的鼻咽癌個案或與高危型的 HPV有關⁵。

鼻咽癌病徵

鼻咽癌的初期可以沒有病徵或症狀並不明顯^1,6。隨著腫瘤的增大並擴散到周圍組織時，就會出現下列症狀：鼻塞、流鼻血或咳嗽帶血、聽力減退、耳鳴或耳道流膿^1,6。鼻咽癌患者可能會出現頸側淋巴核脹大^1,6。如果出現上述情況而病徵持續出現超過2星期，患者應該儘快求醫。

鼻咽癌治療

因鼻咽藏在頭顱中間比較深入的位置，且附近有腦幹等重要組織，醫生要為病人動手術並不容易，因此手術並不是治療鼻咽癌的最首要考慮的方法¹。放射治療和化學藥物治療是醫治鼻咽癌的主要方法。現時的放射治療可精確將輻射劑量集中射向腫瘤以減低輻射對腫瘤周邊正常組織的影響¹。然而，放射治療可引致一些明顯的短期副作用，例如皮膚發紅和痠痛、感到噁心、味覺改變、口乾舌燥和脫髮⁶。

如果腫瘤已擴散至其他器官，醫生也會考慮使用化學藥物治療，因為化學治療可減輕腫瘤引致的症狀和不適¹。在接受化學治療後，鼻咽癌患者可能會覺得噁心和疲倦，還會出現腹瀉和嘴巴疼痛的情況⁶。另外，長時間接受化學治療有可能會出現不育⁶。

鼻咽癌存活率

如果鼻咽癌發現得早，存活率可以很高¹。原位性的鼻咽癌的5年存活率可達82%，若鼻咽癌已經有區域性的轉移，患者的5年存活率仍然可以有72% ⁷。但是，當癌細胞已經遠端轉移，患者的5年存活率會大幅下降至49%。⁷

預防鼻咽癌

要預防鼻咽癌，我們應保持健康的生活習慣：避免醃製和罐頭食品、不要酗酒和吸煙⁵。我們亦應保持警覺並定期自我檢查以預防鼻咽癌，如果發現頸部淋巴結脹大，或出現以上病徵，應該趕緊求醫¹。
